SUMMARY:
The Social Worker is responsible for the counseling of assigned clients as well as their family members. The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, family dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator.
DUTIES PERFORMED:
- Conducts psychosocial history to determine pre-injury issues which need to be addressed.
- Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
- Provides counseling to clients, families and/or significant others.
- Develops and implements family and children support groups as needed.
- Schedules, arranges and moderates the program for family education group.
- Participates in team rounds and family conferences as scheduled through case manger.
- Maintains records and prepare reports according to facility procedures.
- Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
- Performs public relations task as directed by supervisor.
- Coordinates client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist and other disciplines as needed.
- Is an integral part of the interdisciplinary team.
- Assists in patient discharge planning.
- Conducts post discharge follow-up as needed.
- Supervision of Social Work Staff.
- Plays an intricate role in management responsibilities.
- Any other tasks as assigned by the immediate supervisor.
SPECIFICATIONS
- Formal Education/Formal Training: Must hold a Degree in Social Work and applicable license/certification.
- Job Related Experience: Minimum of two years experience in a health care setting, preferably and Inpatient Rehab Facility (IRF)
