Your Impact: As a Physical Therapist in Home Health, you will assess and evaluate patient needs related to functional status, mobility, endurance, and pain management. You will create treatment plans aimed at enhancing patient well-being and evaluate their progress.

Key Responsibilities:
• Evaluate the patient's functional status and PT needs and assist in developing therapy plans.
• Provide services in accordance with state laws and the plan of care, collaborating with the healthcare team.
• Monitor and report patient responses to treatment while communicating changes to the supervising nurse or physician.
• Implement treatment strategies to relieve pain, and restore and maintain maximum function.
• Educate patients, family members, and the healthcare team about the plan of care and progress toward goals.
• Conduct patient visits per the treatment plan, complete progress notes, and document all necessary information as per LHC policy.
• Develop PT plans of care according to state guidelines and report on evaluation outcomes and projected care frequency.
• Adapt to any home situation, regardless of the client's background or condition.

Qualifications:
• Current Physical Therapy licensure in the state of practice.
• Valid driver's license, vehicle insurance, and reliable transportation.

Preferred Qualification:
• Current CPR certification or ability to obtain within 90 days of hire.

Compensation: Annual total cash target pay ranges from $95,000 to $129,910, with hourly rates between $45.67 and $62.46. You'll also earn from a per visit pay structure based on productivity points assigned to various patient visits.
