Port Jervis / Middletown, NY – Full-Time Provider


• NP

• Monday through Friday

• Provider would work between our Port Jervis and Middletown locations

• Full-time schedule with consistent assigned days/locations


Dutchess County, NY – Full-Time Provider


• NP

• Monday through Friday

• Coverage within the Dutchess County area, which may include Poughkeepsie, Wappingers Falls, Beacon, and surrounding locations

• Provider must be comfortable traveling between assigned facilities (although typically never in the same day)


Bronx, NY – Part-Time Provider


• NP

• Preferably two to three shifts per week

• Shifts are generally a minimum of four hours

• There is flexibility regarding which days are worked; however, once a schedule is established, we do expect consistency
